Home and Community Care for people under 65

Support services for younger people

Our Home and Community Care Program for Younger People (HACCPYP) provides basic support services. These are available for people aged under 65 years, or Aboriginal Torres Strait Islander people aged under 50 years.

The program is designed for people who want to live as independently as possible in their homes and community, but who are not eligible for the National Disability Insurance Scheme (NDIS).

Services available

Meals

Have ready-to-eat meals delivered, or have our staff assist you with meal preparation.

Delivered meals

Have tasty and nutritious, ready-to-eat meals delivered to your home. We can cater to your taste preferences and any special dietary requirements. 

Support with meal preparation

Our trained staff can assist by:

  • accompanying you on your grocery shop
  • doing your grocery shop for you based on your shopping list
  • setting up and ordering online groceries through your nearest supermarket (you will need a computer, smartphone or tablet)
  • cutting up ingredients ready for your to cook.

You can even combine both options for a convenient mix of supported meal preparation and delivered meals.

Cost

 $6 per meal.

Personal care

Get some extra support for a range of everyday, personal tasks such as showering, dressing and washing your hair.

Cost

$6 per hour.

Domestic assistance

We can assist you to stay on top of everyday domestic tasks such as general house cleaning and laundry.

Cost

 $6 per hour.

Carer respite

We provide flexible respite during the day to give carers a break from their caring role. This can be in the home or in the community.

Cost

$6 per hour.

Property and garden maintenance

If you need minor household repairs and modifications to help keep you safe and well in your home, we’re ready to help.

We can help with:

  • installing smoke alarms and changing batteries
  • gardening and lawn mowing
  • changing light globes
  • replacing tap washers
  • installing hand rails
  • gutter cleaning
  • installing hand-held showers (with a referral from an occupational therapist).

We can’t help with:

  • constructing ramps
  • concreting, paving or painting
  • spring cleaning
  • activities that require a registered trades person
  • window cleaning.

Cost

$12 per hour.

Community access

We can pick you up from home and accompany you to do your own shopping or other errands such as banking, post office or chemist.

Cost

$1.16 per kilometre.

Linking to local services

We can refer you to community-based servicesonce we have met and completed a living at home assessment.

 

Paying and managing your services

  • Invoices are sent monthly and can be via email or post.
  • Services that occur on public holidays will be automatically suspended.
  • Clients will be provided with an invoice at the end of each month.
  • Non-payment of fees may result in suspension or cancellation of services.
  • Fees will be reviewed in July 2024.
  • Property Maintenance services have a 1-hour minimum service time.
  • The full-service fee is charged where less than 24 hours’ notice of booking cancellation is provided.
  • If you need to go to hospital or are taking a holiday, simply let us know and we can temporarily suspend your services for up to a six-week period.

Accessing the program

A holistic living at home assessment is done to understand each individual’s needs. The assessor will work with you to develop meaningful goals that allow you to live independently in your home and actively participate in your community.
